This presentation highlights observations on a variety of interesting failures and the pertinent engineering lessons that were learned. The failures that are discussed include:
-Buckling of a steel storage silo.
-Collapse and buckling of GRP storage tank roofs.
-Testing to failure of a flexible composite braided umbilical.
-Post-buckling comparison of simply-supported carbon composite plates under uniaxial compression, with various cut-out geometries.
-An investigation into the failure of rotationally moulded oil LLDPE storage tanks.
-Buckling of the leaflets in a polyurethane heart valve.

The presentation will reflects on relevant simulation issues as well as engineering lessons.
